1. About this agreement
This Parent and Guardian Agreement (the agreement) is between the parent or legal guardian signing below (you) and Silly Goose Social Club. It applies to your child's participation in all Silly Goose Social Club programs, including the weekly Friday Social Club and Drop-In Fridays held at our studio located at 625 Main Street, Windermere, FL 34786, Storybook Date Nights, and in-home experiences such as Storybook Birthday Experiences and the Big Sibling Club.
Please read it completely and carefully before your child participates. By signing, you confirm that you are the parent or legal guardian of the child or children named below and that you have the authority to enter into this agreement on their behalf.
2. About our programs
Silly Goose Social Club provides screen-free, play-based enrichment led by early childhood educators. Activities are designed for children and may include storytelling, imaginative and open-ended play, process art, sensory experiences, music and movement, simple science invitations, games, and supervised outdoor play.
Silly Goose Social Club is an enrichment experience. It is not childcare, preschool, tutoring, medical care, or therapy. Our educators are not medical professionals.
3. Health and medical acknowledgment
You confirm that your child is in good health and is physically, emotionally, and developmentally able to participate in the activities described above. You agree to disclose, in the information section below, any allergies, medical conditions, medications, sensory sensitivities, developmental considerations, or recent injuries that we should know about.
You agree to keep this information current and to notify us promptly of any changes. You understand that our educators are not medical professionals and do not diagnose, treat, or administer medication.
4. Emergency medical authorization
In the event of an injury, illness, or medical emergency, you authorize Silly Goose Social Club to take reasonable steps to care for your child, including contacting you and your listed emergency contacts and, if you cannot be reached and it is necessary, calling 911 or seeking emergency medical care.
You agree to be financially responsible for any medical care, transportation, or treatment your child receives. You understand that Silly Goose Social Club is not responsible for the cost of such care.
5. Emergency contacts and authorized pickup
For the Friday Social Club and any drop-off program, your child will be released only to you or to an adult you have authorized in writing in the information section below. We may ask for photo identification before releasing a child. Please notify us in advance of any change to authorized pickup arrangements.
For in-home experiences, such as Storybook Birthday Experiences and the Big Sibling Club, you agree that a parent or an approved adult will remain reachable by phone for the entire experience.
6. Illness and communicable disease policy
To protect every child, please keep your child home if they are unwell. Your child should not attend if, within the previous 24 hours, they have had a fever of 100.4 degrees or higher, vomiting, diarrhea, an undiagnosed rash, or a contagious illness, or if they are otherwise too unwell to comfortably take part.
If a child becomes ill during a program, we will contact you for prompt pickup. You acknowledge that group settings carry an inherent risk of exposure to common childhood illnesses, and you assume that risk.
7. Allergies, materials, food, and outdoor play
Our activities use age-appropriate art supplies, sensory materials, and craft items, and may involve common materials or foods that can cause allergic reactions. You agree to disclose all known allergies and sensitivities below. If your child has a severe or life-threatening allergy, please tell us before the first visit so we can plan together.
You consent to your child taking part in supervised outdoor play when offered, and to the reasonable application of sun protection if you provide it or authorize it below.
8. Supervision and comfort
We keep groups small and child-led. Our educators follow each child's comfort level and never force participation. We are not able to provide one-to-one medical, behavioral, or special-needs supervision unless arranged in advance in writing.
9. Behavior and participation
For the safety and enjoyment of all children, you agree that your child will be guided to treat others, the educators, and the space and materials with care. If a child's behavior repeatedly endangers themselves or others, or cannot be safely managed in the setting, we may ask that the child be picked up and may, at our discretion, end participation. We use gentle, positive guidance and never use physical or harsh discipline.
10. Photo, video, and media release (optional)
With your permission, we love to capture the joy of play. If you opt in below, you grant Silly Goose Social Club permission to photograph and record your child and to use those images and recordings, without compensation, for promotional and educational purposes, including our website and social media. This permission is entirely optional and you may decline or withdraw it at any time in writing, and we will not use new images of your child going forward.
11. Assumption of risk
You understand that participation in play, movement, art, sensory, and outdoor activities involves inherent risks, including the risk of minor bumps, scrapes, falls, allergic reactions, exposure to common illnesses, and, though unlikely, more serious injury. Knowing this, you voluntarily accept and assume all such inherent risks of your child's participation.
12. Release and hold harmless
To the fullest extent permitted by law, you, on behalf of yourself, your child, and your and their heirs and representatives, release, waive, and agree to hold harmless Silly Goose Social Club (a trade name of [Windermere Nannies, LLC]), together with its owners, employees, contractors, and agents, including Ms. Goose (Katelyn) and Ms. Swan (Amanda), from any and all claims, demands, or causes of action for personal injury, illness, or property damage arising out of or related to your child's participation that result from the inherent risks of the activities. This release does not apply to injury or damage caused by gross negligence or intentional misconduct.
13. Indemnification
To the fullest extent permitted by law, you agree to indemnify and hold harmless Silly Goose Social Club (a trade name of [Windermere Nannies, LLC]), together with its owners, employees, contractors, and agents, including Ms. Goose (Katelyn) and Ms. Swan (Amanda), from any claims, liabilities, damages, or expenses, including reasonable attorney fees, brought by a third party and arising out of your child's participation or your breach of this agreement, except to the extent caused by gross negligence or intentional misconduct.
14. Notice to the minor child’s natural guardian
READ THIS FORM COMPLETELY AND CAREFULLY. YOU ARE AGREEING TO LET YOUR MINOR CHILD ENGAGE IN A POTENTIALLY DANGEROUS ACTIVITY. YOU ARE AGREEING THAT, EVEN IF SILLY GOOSE SOCIAL CLUB USES REASONABLE CARE IN PROVIDING THIS ACTIVITY, THERE IS A CHANCE YOUR CHILD MAY BE SERIOUSLY INJURED OR KILLED BY PARTICIPATING IN THIS ACTIVITY BECAUSE THERE ARE CERTAIN DANGERS INHERENT IN THE ACTIVITY WHICH CANNOT BE AVOIDED OR ELIMINATED. BY SIGNING THIS FORM YOU ARE GIVING UP YOUR CHILD’S RIGHT AND YOUR RIGHT TO RECOVER FROM SILLY GOOSE SOCIAL CLUB IN A LAWSUIT FOR ANY PERSONAL INJURY, INCLUDING DEATH, TO YOUR CHILD OR ANY PROPERTY DAMAGE THAT RESULTS FROM THE RISKS THAT ARE A NATURAL PART OF THE ACTIVITY. YOU HAVE THE RIGHT TO REFUSE TO SIGN THIS FORM, AND SILLY GOOSE SOCIAL CLUB HAS THE RIGHT TO REFUSE TO LET YOUR CHILD PARTICIPATE IF YOU DO NOT SIGN THIS FORM.
15. Governing law and other terms
This agreement is governed by the laws of the State of Florida. If any part of this agreement is found unenforceable, the remaining parts stay in full effect. This agreement is the entire understanding between you and Silly Goose Social Club about its subject and remains valid for the duration of your child's participation unless revoked in writing.
16. Electronic records and signature
You agree that this agreement may be signed and stored electronically, and that an electronic signature has the same legal effect as a handwritten one. You consent to receive related records electronically.
